Comparing Accelerated Partial Breast Irradiation using Intensity-Modulated Radiotherapy with Whole Breast Irradiation: The Florence Experience

Summary: Breast-conserving therapy with radiation has been found to be as effective as mastectomy in overall survival and can reduce recurrence and mortality rates. Hypofractionated whole-breast irradiation can shorten treatment time without compromising safety. Partial-breast irradiation (PBI), including Brachytherapy, Intraoperative RT, and external beam radiotherapy (EBRT), is an option for selected patients with unique pros and cons. The EBRT accelerated partial breast irradiation (APBI) intensity-modulated radiotherapy (IMRT) Florence trial has shown positive results and long-term follow-up.

Dr. Salvestrini will highlight the crucial role of patient selection for PBI which should follow ESTRO, ASTRO and recent consensus recommendations. To define future directions, the pre-and post-operative multidisciplinary discussion should be mandatory for all breast cancer patients, biology should be definitively integrated into the decision-making process and radiation oncologists should always consider breast cancer as a heterogeneous disease. It is not a “one treatment fits all” disease.
